Chronic Disease Management

Chronic disease management is the structured, ongoing care of long term conditions such as diabetes, asthma, heart disease and arthritis. Instead of reactive visits, care follows a plan: scheduled reviews, monitoring, medication adjustments and coordinated referrals to allied health.

Medicare supports formal GP chronic disease management plans, which can unlock rebated allied health visits for eligible patients. Telehealth suits the regular review rhythm of chronic care well, reducing travel for patients managing conditions over years.
